Brisbane South PHN works in partnership with our local communities and health care providers to improve health and wellbeing across the Brisbane south region. We are committed to creating a better system for you, so that you can have the best health possible.

We serve the Brisbane south community through supporting primary health care, which includes services provided by general practice, allied health services, community health and community pharmacies.

Mental Health

Brisbane South PHN commissions a number of services for people experiencing all different levels of mental health issues.  Our services fit within what we call a ‘stepped care model’, ranging from mild depression and anxiety through to severe, complex or crisis mental illness.

Aboriginal and Torres Strait Islander Health

Brisbane South PHN is committed to improving the health of Aboriginal and Torres Strait Islander Peoples and supporting culturally sensitive care. If you want to improve your physical, mental, spiritual and cultural wellbeing, we have programs to help.

Advance care planning

If something was to happen to you, would your loved ones know what care you would want and be able to speak on your behalf? Advance care planning gives you the opportunity to discuss your plans for care and treatment, should you be unable to make or communicate those decisions in the future. Even if you’re fit and healthy, it’s never too early to plan your future health care.

My health for life

People are often unaware of their risk of developing future health conditions because they feel okay or don’t show any symptoms. Check your risk of chronic disease today with My health for life! You may be eligible for the free My health for life program, a six session course designed to help you get your health back on track.
